Complete the RealAge test by February 20, 2020

NEW TO THE WELLNESS PROGRAM?
Beginning January 6, 2020, go to clh.sharecare.com and follow the steps below to complete the RealAge test.

NEW USERS

1. When you are ready to begin the RealAge test, read the instructions below and then click the red TO CREATE A NEW ACCOUNT AND BEGIN THE REALAGE TEST button below.

2. Click the CREATE YOUR ACCOUNT button in the top right-hand corner. (It is important to confirm you are on the correct website. The image in the background on this page should be the Buckingham Fountain.)



3. Enter your first and last name, date of birth, zip code and gender. In the Member ID field at the bottom of the page, enter the nine-digit identification number found in the top left corner of your BCBS identification card. DO NOT enter the 3 letters at the beginning - only the nine numbers. Click NEXT.

4. Choose your login credentials by entering a unique email address and password. Click NEXT.

5. Select your preferences. Once completed, click COMPLETE ACCOUNT CREATION.

a. If you wish to retain your biometric information should you leave the program, confirm by checking the box under HIPAA.

b. If you wish to receive email reminders on wellness program deadlines and participation requirements, confirm that the box under Messaging Preferences is checked.



6. Click CONTINUE to skip the Sharecare app page.

7. On the next page, click the green GET STARTED button to begin your test







WHAT IF I DON’T COMPLETE THE REALAGE TEST?
If you or your covered spouse, domestic partner or civil union spouse do not complete the RealAge test by February 20, 2020, the employee will incur a $50 per non-participant increase in monthly health care contributions for each individual who does not complete the RealAge test.
