Everyone enrolled in the Chicago Lives Healthy wellness program MUST complete a Biometric Screening each year. The deadline to submit all Biometric Screening results is 11:59 pm CST on March 8, 2015.

WHAT IS A BIOMETRIC SCREENING?

A biometric screening is a quick and easy check-up that can help identify health issues before they become detrimental. A health care professional collects your height and weight measurements, waist measurement and blood pressure. Then he or she administers a small finger-stick blood test measuring cholesterol, triglycerides and glucose levels.

You’ll receive your results immediately following the biometric screening. Make sure to keep a paper copy of your results.

WHAT OPTIONS DO I HAVE FOR COMPLETING MY BIOMETRIC SCREENING?

You can complete your biometric screening by choosing one the three options listed below:

A. ONSITE BIOMETRIC SCREENINGS

Beginning December 15, 2014 you can schedule your free onsite biometric screening at select work locations, park facilities and community locations. If you choose to complete an onsite screening during work hours, please consult with your supervisor prior to scheduling to ensure proper coverage. Please note: Onsite Biometric Screenings begin on January 15, 2015 and all results must be received by March 8, 2015.

B. PHYSICIAN OFFICE BIOMETRIC SCREENING

You can also go to your physician’s office to complete your biometric screening. Biometric screenings done at your physician’s office will be subject to your current medical plan benefits.

Personalized Physician Forms are available beginning December 15, 2014. Click the button below and enter your legal first name, legal last name, date of birth, gender and agency to obtain your Personalized Physician Form. Take it with you to your screening appointment at your physician’s office. Healthways will accept biometric screening results if the test results are no more than six months old (no results collected prior to 6/15/2014 will be accepted).

Please note: Biometric screening results from your physician faxed in on your personalized physician form must be received by March 8, 2014. If your physician submits biometric results on any other type of form, the results cannot be accepted.

C. WALGREENS PHARMACIES

Beginning January 15, 2015 through March 8, 2015 you can obtain a personalized Walgreens Health Test Voucher and make an appointment at your local Walgreens Pharmacy to complete a free biometric screening. Click here for a list of participating Walgreens Pharmacies. You must bring:

  • Your personalized Walgreens Health Test Voucher
  • A photo ID

Please note: If you are required to pay for your biometric tests or use your healthcare insurance at Walgreens, you will not be reimbursed for any out of pocket expenses.

WHAT IF I DON’T COMPLETE A BIOMETRIC SCREENING?

If you and/or your covered spouse/domestic partner/civil union spouse do not complete a biometric screening by March 8, 2015, you will incur a $50 per non-participant increase in your monthly employee health care contribution.

Step 2 COMPLETE A WELL-BEING ASSESSMENT (WBA)

Everyone enrolled in the Chicago Lives Healthy wellness program MUST complete a WBA each year. You have from January 15, 2015 through 11:59pm CST on March 31, 2015 to complete your online WBA for the 2015 plan year.

The Well-Being Assessment (WBA) is a confidential questionnaire that assesses life and environmental factors that are critical to your health and well-being and your ability to improve both. The Well-Being Assessment (WBA) has been designed by health experts and consists of 60 questions to help get a better sense of factors such as:

  • Your basic health history
  • Your mental health and stress, and
  • Any barriers to change that might keep you from moving forward

All questions are simple to understand and easy to answer. It should take 20 to 30 minutes to complete the questionnaire.

Do you need help completing your Well-Being Assessment (WBA)?

Health coaches in the Chicago Lives Healthy wellness program are available to help you complete your Well-Being Assessment on the computer in a one-on-one session or in a classroom setting. Below are 2 options to sign up for a Well-Being Help Session:

  1. Click here to sign up for a one-on-one or a classroom Well-Being Help Session.

  3. Call Healthways at 1-866-556-7671 and sign-up for a Well-Being Help Session through a customer service representative.

Important Information:

1. Please complete the online WBA even if your biometric results are not present. Once you begin the WBA, if your biometric results are not present, you will be prompted to input your height and weight measurements in the boxes and then continue to the next page. When your biometric results are received, your WBA and your Well-Being report will be updated with your results. Entering your biometric results does not satisfy the requirement to obtain a screening.

2. RETURNING USERS: Log on to WBC, using your registered username and password. You will then be prompted to complete the WBA. Upon completion of the WBA, you will immediately receive your well-being report. Click the “Continue to Well-Being Plan” button to access all benefits of WBC.

3. NEW USERS: Go to WBC and click the “Sign Up” button. You will be prompted to register and create a username and password. You will then be prompted to complete the WBA. Following completion of the WBA, you will be asked to provide your email address. You will be sent an email once the WBA is processed, typically within 24 hours. Upon receipt of this email you can create your Well-Being Plan by accessing Well-Being Connect (WBC) to explore and discover how WBC can help you set and achieve your health goals.

Step 3 TAKE THE CALL FROM YOUR HEALTH ADVISOR

Everyone enrolled in the Chicago Lives Healthy wellness program MUST complete a Health Advisor Check-In Call each year. You have from January 15, 2015 through 11:59 pm CST on March 31,2015 to complete a your Health Check-In Call.

WHAT IS THE HEALTH ADVISOR CHECK-IN CALL?

The Health Advisor Check-In Call is an outbound call to you from one of Healthways Health Advisors who are certified health professionals. During this 15 minute call, the Health Advisor will review your biometric screening and your Well-Being Assessment results with you. The Health Advisor will talk with you about your health and well-being and if you wish, develop realistic health or lifestyle improvement goals with you.

Please update your primary phone number on Well-Being Connect so that the Healthways Health Advisor can contact you at the correct phone number to complete the Health Check-In Call.

Access WBC to update your contact information

IMPORTANT INFORMATION ABOUT YOUR HEALTH ADVISOR CHECK-IN CALL:

1. You will get a call from the Healthways Health Advisor regarding your Health Check-In once you have completed both your biometric screening and your Well-Being Assessment.

2. A separate Health Check-In call will be made to your covered spouse/domestic partner/civil union spouse by Healthways Health Advisor once they complete their biometric screening and WBA.

3. When you complete your Health Check-In Call, you will also earn 6 participation points towards the Quarter 2 participation requirement. Quarter 2 begins on 4/1/2015!

4. If you have completed your biometric screening and your online Well-Being Assessment and have not yet received a call from a Healthways Health Advisor, you can call 1-866-556-7671 and press 2 to speak to a Health Advisor.

IMPORTANT INFORMATION:

Healthways Health Advisors will make three call attempts to reach you over a 90 day period at the phone number you listed on the Well Being Connect portal to complete your Health Check-In Call. If 3 calls have been made to you and you have not responded to the calls, Healthways will send you a letter to let you know that must contact a Health Advisor to complete your Health Check-In Call. Health Advisors are available at 1-866-556-7671 and press prompt 2.

EARN 12 WELLNESS POINTS EACH CALENDAR QUARTER

FIRST QUARTERLY POINTS REQUIREMENT RUNS FROM APRIL 1, 2015 THROUGH JUNE 30, 2015

Please note: You do not have to earn points in the first quarter because you are completing your three enrollment steps: your biometric screening, your well-being assessment and your health advisor check-in call.

Beginning on April 1, 2015, the start of the second quarter all participants in the Chicago Lives Healthy wellness program will be required to earn 12 new points each quarter by completing wellness program activities.

Complete Well-Being Connect online activities and earn 1 point for each activity completed.

Well Being Connect™ (WBC) is a dynamic online portal designed to inspire improved health and well-being. Every personalized feature is meant to help you live more fully by getting to know you better, helping you set goals and connecting you to support to achieve them. Well Being Connect™ is a convenient and effective way to earn 12 points at one time if you wish.

ACCESS WELL-BEING CONNECT

Attend an Onsite Health Education Session, and earn 6 points.

One of the best parts of the Chicago Lives Healthy wellness program is access to free, top-quality health education sessions for participants. Whether you’re setting goals to lose weight, managing ongoing lifestyle issues like quitting smoking or just want to learn more about the Chicago Lives Healthy wellness program, a health education session allows you to focus specifically on your questions, needs and goals with a health coach. These short, informative sessions cover a variety of topics to help you live your best.

ACCESS ONSITE HEALTH EDUCATION SESSIONS

Participate in local healthy activity events and earn 6 points.

Chicago Lives Healthy recognizes that local and community activities, such as organized runs and walks, are great ways to improve your health and well-being. Learn more by clicking the ACCESS HEALTHY ACTIVITY button below.

ACCESS HEALTHY ACTIVITY

Complete a Telephonic Coaching Session, and earn 6 points.

(Eligibility for telephonic coaching is determined by a participant’s biometric screening and Well-Being Assessment results. Not all participants will qualify for telephonic coaching.)

Telephonic health coaching is a voluntary program that provides personal support from a health coach to help individuals with moderate to high health risk factors overcome the challenges that stand in the way of their good health. Based on your responses to the questions you completed on the Well Being Assessment™ and the values presented from your biometric screening you may be offered an opportunity to participate in the telephonic health coaching program when you complete Step 3, the Health Advisor Check-In Call.

ADDITIONAL OPTION AVAILABLE FOR CITY OF CHICAGO PPO PARTICIPANTS ONLY:

Participate in a Care Management Program, and earn 12 points per quarter.
