What is a Biometric Screening?

A biometric screening includes tests to measure your cholesterol, triglyceride and glucose levels. During your biometric screening appointment, a health care professional will also determine your blood pressure, height, weight and waist measurements.

What options do I have for completing my Biometric Screening? *

You can complete your biometric screening in one of two ways:

A. Schedule an appointment and attend an onsite screening at a community or work location such as City Hall or select Chicago Public Libraries or select Chicago Park District facilities; OR,

B. Schedule and attend an appointment with your doctor.

Beginning on December 1, 2017, you can also schedule an appointment with your physician’s office to complete your biometric screening. Follow these steps below:

1. Call your doctor to schedule an appointment.

2. Download the Personalized Physician Form. Bring your Personalized Physician Form with you to your doctor's office.

3. Let your doctor know the Personalized Physician Form must be completed and faxed to 615-349-2344 by March 10, 2018.

Here is some additional Information about Biometric Screenings completed in your doctor's office:

• Healthways will accept biometric screening results if the test results are no more than six months old (no results collected prior to July 1, 2017 will be accepted).

• Biometric screenings done at your doctor’s office are subject to your current medical plan benefits. HMO members must see their Primary Care Physician (PCP) and PPO members must use an in-network provider.

• Your biometric screening results must be submitted on your Personalized Physician Form. If your physician submits biometric results on any other type of form, the results cannot be accepted and your enrollment Step 1 will be considered incomplete.

STEP 2:

COMPLETE A WELL-BEING 5 ASSESSMENT (WB5) by March 10, 2018

All eligible employees and covered spouses/domestic partners/civil union spouses must complete a Well-Being 5 Assessment (WB5). You have from January 4, 2018 through March 10, 2018 to complete your online WB5 for the 2018 plan year.

The Well-Being 5 Assessment (WB5) measures your current well-being and is used to help you and your Health Advisor identify potential areas of focus for your personal 2018 Wellness Program goals.

The WB5 is a simple, confidential questionnaire that looks at your lifestyle habits and current health status to see what may be helping or hurting your overall well-being. You will answer a few focused questions and get instant feedback in your personal Well-Being 5 Snapshot, including suggestions on ways to increase your well-being.

The questions are simple to understand and easy to answer. It will take about 15 minutes to complete the questionnaire. You can complete your online WB5 prior to completion of your biometric screening or before your biometric screening results are processed and recorded on Well-Being Connect.

Important Information:

1. Please complete the online WB5 even if your biometric results are not present on Well Being Connect. Once you begin the WB5, if your biometric screening results are not present, you will be prompted to input your height and weight measurements and then continue to the next page. When your biometric screening results are processed, your WB5 and your Well-Being 5 Snapshot will be updated with your results. Entering your biometric screening results in the WB5 does not satisfy the requirement to obtain a screening.

2. RETURNING USERS: Log on to Well-Being Connect (WBC), using your registered username and password. You will then be prompted to complete the WB5. When you click the Finish button, you will immediately receive your Well-Being 5 Snapshot.

2. NEW USERS: Go to WBC and click the “Sign Up” button. You will be prompted to register and create a username and password. Please document your username and password as you will need this information to access Well-Being Connect in the future. You will then be prompted to complete the WB5. When you click the Finish button, your assessment is complete and your Well-Being 5 Snapshot will be available. Within 24 hours you will receive an email indicating you have access to Well-Being Connect to create your Well-Being Plan to help you set and achieve your health goals. When your actual biometric screening data is loaded, you will be notified via email and you can log in to review your new Well-Being 5 Snapshot.

Do you need help completing your Well-Being 5 Assessment (WB5)?

STEP 3:

TAKE THE CALL FROM A HEALTH ADVISOR by April 30,2018

All eligible employees and covered spouses/domestic partners/civil union spouses must complete a Health Advisor Check-In Call by April 30, 2018. A Healthways Health Advisor will call you when they have the results from your biometric screening and your WB5 on file for 2018. Health Advisor calls can be completed from January 4, 2018 through April 30, 2018.

WHAT IS THE HEALTH ADVISOR CHECK-IN CALL?

The Health Advisor Check-In Call is an outbound call to you from one of Healthways' certified health professionals. During this 15 minute call, the Health Advisor will review your biometric screening and your Well-Being 5 Assessment results (WB5) with you. The Health Advisor will talk with you about your health and well-being and if you wish, develop realistic health or lifestyle improvement goals with you.

Please update your primary phone number in Well-Being Connect so that the Healthways Health Coach can call you to complete the Health Advisor Check-In Call.

IMPORTANT INFORMATION ABOUT YOUR HEALTH ADVISOR CHECK-IN CALL:

1. Individual Health Advisor Check-In calls will be made to each wellness participant.

2. If you have completed your biometric screening and your online Well-Being 5 Assessment and do not receive a call from a Healthways Health Advisor, please call 1-866-556-7671. Please also check to see if the phone number that you have provided is correct.

IMPORTANT INFORMATION:

Healthways' Health Advisors will call you at the phone number you listed in the Well Being Connect portal to complete your Health Check-In Call. Three outbound call attempts will be made to you by Healthways. If Healthways’ Health Advisors have called you three times with no response, they will send you a letter indicating that you MUST contact a Health Advisor to complete your Health Advisor Check-In Call. Call Healthways at 1-866-556-7671 to complete your Health Advisor Check-In Call before April 30, 2018.

STEP 4:

ONGOING PARTICIPATION

There are 2 ongoing participation paths. For some participants, ongoing participation is REQUIRED and for others ongoing participation is OPTIONAL. The Chicago Lives Healthy wellness program no longer requires any participant to earn quarterly participation points; however, participants who are required to be in a Health Improvement Program (HIP) must complete required activities on a quarterly basis.

How were participants selected for REQUIRED Health Improvement Program (HIP) participation?

Participants were assigned to a Health Improvement Program (HIP) based on a combination of factors including biometric screening data and, for PPO participants, medical claim data

• Participants who have 4 or 5 markers for Metabolic Syndrome.
Metabolic Syndrome is the name for a group of risk factors that raises your risk for heart disease and other health problems, such as diabetes and stroke. You can have any one of these risk factors by itself, but they tend to occur together. Participants who have four or five metabolic risk factors or markers will be included in this category. In accordance with the National Institutes of Health guidelines, the risk factors or markers for Metabolic Syndrome include:

> High triglyceride level — 150 milligrams per deciliter (mg/dL), or 1.7 millimoles per liter (mmol/L), or higher of this type of fat found in blood
> Reduced high-density lipoprotein (HDL) cholesterol — less than 40 mg/dL (1.04 mmol/L) of this “good” cholesterol
> Increased blood pressure — 130/85 millimeters of mercury (mm Hg) or higher
> Elevated fasting blood sugar — 100 mg/dL (5.6 mmol/L) or higher
> Large waist circumference — a waistline that measures at least 35 inches (89 centimeters) for women and 40 inches (102 centimeters) for men

Participants who are identified as likely to need significant healthcare resources or have worsening of an existing health condition during the coming year.
Predictive modeling software was used to identify those participants in higher risk categories. Condition Management services can help these participants by coordinating healthcare with their doctors, and educating the participant on managing his or her conditions. Some of the conditions requiring care management or disease management services are:

> Cardiac
> COPD
> Diabetes
> Chronic Kidney Disease
> Asthma
> Depression & Obesity (interwoven into all programs as applicable)
> Multiple chronic conditions

How are participants notified to enroll in the REQUIRED Health Improvement Program (HIP)?

If you are REQUIRED to participate in a Health Improvement Program you received information regarding your (HIP) assignment in the Chicago Lives Healthy wellness program launch letter dated January 5, 2018 from the Chicago Benefits Office. The Health Improvement Program (HIP) vendors Telligen, and TCOYH are communicating by sending written correspondence and making telephone calls encouraging REQUIRED participants to enroll by April 30, 2018.

What is the deadline for initial enrollment in my REQUIRED Health Improvement Program (HIP)?

Initial enrollment for all Health Improvement Programs offered by Telligen or TCOYH must be completed by April 30, 2018.

Where can I go to determine if I have completed my REQUIRED active quarterly participation?

If you have been assigned to a Health Improvement Program (HIP) the quarterly point requirement has been replaced by an “active participation” requirement. The active participation requirement will differ by program. Following your initial enrollment session, individualized coaching sessions will be agreed upon and communicated to you for each quarter by your health coach.

The quarterly active participation dates are:

• April 1, 2018 to June 30, 2018
• July 1, 2018 to September 30, 2018
• October 1, 2018 to December 31, 2018

When you have met the program goals agreed upon for each quarter, a “green star” will appear on the rewards center tab, in the “My Progress” column next to, “Complete Quarter 2 Requirement”, in the HEALTHWAYS Well-Being Connect (WBC) portal. Here is an example of what your rewards center will look like if you have completed the ongoing requirement:

Health Improvement Programs: Here is a brief listing of the Health Improvement Programs that are offered to those persons whose participation is REQUIRED in 2018. Please note: Your active participation in any of the Health Improvement Programs (HIP) does not require that you meet any physical performance goals such as a reduction in blood pressure, weight or cholesterol readings.

• CONDITION MANAGEMENT PROGRAM
• TAKING CONTROL OF YOUR HEALTH (TCOYH)

METABOLIC SYNDROME COACHING PROGRAM

Telligen is a long term partner with the City of Chicago and is dedicated to the improvement of member’s quality of life through relationship based health coaching. The goal is to assist the member with improving current or potential medical conditions to live a healthy, active life. The dedicated health coach will assist the member with development of a personal health improvement action plan tailored to their unique risk factors and provide strategies, education and support to the member while making progress toward improving their health.

CONDITION MANAGEMENT PROGRAM

The Telligen health professional will coordinate the participant’s healthcare treatments, engage medical providers on evidence-based practices and educate and encourage the participant’s self-management skills regarding their health conditions.

TAKING CONTROL OF YOUR HEALTH (TCOYH)

Since 2009, The Illinois Pharmacists Association (IPhA) and the Midwest Business Group on Health (MBGH) have partnered to provide employers and their covered employees with the chronic disease management program, Taking Control of Your Health (TCOYH).

The program is designed to increase your knowledge and understanding of your condition while learning about proper diet, exercise and stress management techniques to adopt healthy behaviors to manage and improve your diabetes. Participants in this program will have private, face to face counseling sessions with a personal diabetes coach, a specially trained pharmacist who works closely with the participant and their physician, to learn how to better manage and monitor their chronic conditions (diabetes, high blood pressure, cholesterol), and support the effective use of their medications.

What if I choose not to participate in my REQUIRED Health Improvement Program (HIP)?

If you choose not to participate in your REQUIRED Health Improvement Program, the City of Chicago employee will incur a $50 per non-participant increase in his/her monthly health care contribution.

Participation is Optional: If you have been assigned to the “optional participation” requirement, you may continue to use the Well-Being Connect portal, participate in on-site education sessions, or work with a Healthways’ Health Coach. The Chicago Lives Healthy wellness program will no longer require participants to earn quarterly participation points.

Onsite Health Education Sessions

One of the best parts of the Chicago Lives Healthy wellness program is access to free, top-quality health education sessions for participants. Whether you’re setting goals to lose weight, managing an ongoing lifestyle issues like quitting smoking or just want to learn more about the Chicago Lives Healthy wellness program, a health education session allows you to focus specifically on your questions, needs and goals with a health coach.

Voluntary Telephonic Coaching Sessions

Eligibility for telephonic coaching with Healthways is determined by a participant’s biometric screening and Well-Being 5 Assessment results. Telephonic health coaching is a voluntary program that provides personal support from a health coach to help individuals with moderate to high health risk factors overcome the challenges that stand in the way of their good health. Telephonic coaching with Healthways is not available to people who are REQUIRED to participate in a Health Improvement Program (HIP) program.

How participants were notified that their ongoing participation is OPTIONAL?

If ongoing participation is OPTIONAL for you for the 2018 wellness program year, this information was communicated to you in the Chicago Lives Healthy wellness program launch letter dated January 5, 2018 sent by the Chicago Benefits Office. If you are unable to locate that letter and you are uncertain about your ongoing participation status for the 2018 program year, you can also confirm your status by:

1) Confirming that there is a green star in the “My Progress” column in the rewards center tab, next to the “Voluntary Ongoing Participation” section. See screenshot below showing what it looks like;

What tools are available to me in the Chicago Lives Healthy wellness program if my ongoing participation is OPTIONAL?

You may continue to use the Well-Being Connect portal, participate in on-site education sessions, or work with a Healthways’ Health Coach. If you are enrolled in the Blue Choice Opt PPO Plan you may also be eligible to participate in a disease or care management program offered through the health plan.
