Everyone enrolled in the Chicago Lives Healthy wellness program MUST complete a WB5. You have until February 29, 2016 to complete your online WB5 for the 2016 plan year.

The Well-Being 5 Assessment (WB5) measures your current well-being and is used to help you and your Health Advisor identify potential areas of focus for your personal 2016 Wellness Program goals.

The WB5 is a simple, confidential questionnaire that looks at your lifestyle habits and current health status to see what may be helping or hurting your overall well-being. You will answer a few focused questions and get instant feedback in your personal Well-Being Report, including suggestions on ways to increase your well-being.

All questions are simple to understand and easy to answer. It should take 15 minutes to complete the questionnaire.

1. RETURNING USERS: Log on to Well-Being Connect (WBC), using your registered username and password. You will immediately be prompted to complete the WB5. If your biometric screening results are not present, please provide as many lab test results and biometric information as you can and continue to the next page. Upon completion of the WB5, you will immediately receive your well-being report.

2. NEW USERS: Go to WBC and click the “Sign Up” button. You will be prompted to register and create a username and password. You will then be prompted to complete the WB5. Once you begin the WB5 please provide as many lab test results and biometric information as you can and continue to the next page. (When your actual biometric screening data is available, you will be notified via email and you can review your new Well-Being Report.) Following completion of the WB5, you will be asked to provide your email address. Within 24 hours your WB5 results will be processed and you will be sent an email. Upon receipt of this email you can access WBC and create your Well-Being Plan to help you set and achieve your health goals.

Do you need help completing your Well-Being 5 Assessment (WB5)?

Health coaches in the Chicago Lives Healthy wellness program are available to help you complete your Well-Being 5 Assessment on a computer. Below are 2 options to sign up for a Well-Being Help Session:

WHAT IF I DON’T COMPLETE THE WBA?

The Healthways Health Advisor will call you as soon as they have the results from your biometric screening and your WB5 on file for 2016. Health Advisor calls can be completed from January 15, 2016 through March 31, 2016.

WHAT IS THE HEALTH ADVISOR CHECK-IN CALL?

The Health Advisor Check-In Call is an outbound call to you from one of Healthways Health Coaches who are certified health professionals. During this 15 minute call, the Health Advisor will review your biometric screening and your Well-Being 5 Assessment results (WB5) with you. The Health Advisor will talk with you about your health and well-being and if you wish, develop realistic health or lifestyle improvement goals with you.

Please update your primary phone number in Well-Being Connect so that the Healthways Health Coach can call you to complete the Health Advisor Check-In Call.

IMPORTANT INFORMATION ABOUT YOUR HEALTH ADVISOR CHECK-IN CALL:

1. Individual Health Advisor Check-In calls will be made to your covered employees and their covered spouse/domestic partner/civil union spouses.

2. For those of you that were not required to get a biometric screening in 2016, your 2015 biometric results and your 2016 Well-Being 5 Assessment (WB5) results will be discussed when you complete your Health Advisor Check-In Call.

3. You will earn 6 quarterly participation points towards the Quarter 2 participation requirement when your complete your Health Advisor Check-In Call. Quarterly participation requirements begins on April 1, 2016!

4. If you have completed your biometric screening and your online Well-Being 5 Assessment and do not receive a call from a Healthways Health Advisor, please call 1-866-556-7671. Please also check to see if the phone number that you have provided is correct.

ADDITIONAL OPTION AVAILABLE FOR CITY OF CHICAGO PPO PARTICIPANTS ONLY:

If you have a chronic health condition such as diabetes, cardiac disease, asthma, COPD or your biometric screening results indicate that you are at high risk of developing these conditions, the Health Advisor may suggest that you are a good candidate to participate in one of the care management programs offered to PPO participants. These health management programs include Taking Control of Your Health, the Telligen Condition Management programs or the Telligen Coaching program. You can find more information about these program under the Quarterly Participation section of the Chicago Lives Healthy website, look under; Participate in a Care Management Program.

IMPORTANT INFORMATION:

Healthways Health Advisors will call you at the phone number you listed in the Well Being Connect portal to complete your Health Check-In Call three different times. All participants in the Chicago Lives Healthy program will be required to earn 12 new points during each quarter starting with the second quarter of 2016 (April through June, 2016) by completing wellness program activities. You can earn all 12 quarterly points at one time if you wish!

Here are the ways that you can earn participation points:

Complete Well-Being Connect online activities and earn 1 point for each activity completed.

Well Being Connect™ (WBC) is a dynamic online portal designed to inspire improved health and well-being. The online portal will help you live more fully by getting to know you better, helping you set goals and connecting you to support to achieve your goals. Well Being Connect™ is a convenient and effective way to earn 12 points at one time if you wish. For example, you can track your exercise or food use, create a personal journal to record your thoughts and actions about your health or read recipes and articles tailored to your personal needs.

ACCESS WELL-BEING CONNECT

Attend an Onsite Health Education Session, and earn 6 points.

One of the best parts of the Chicago Lives Healthy wellness program is access to free, top-quality health education sessions for participants. Whether you’re setting goals to lose weight, managing an ongoing lifestyle issues like quitting smoking or just want to learn more about the Chicago Lives Healthy wellness program, a health education session allows you to focus specifically on your questions, needs and goals with a health coach. These short, informative sessions cover a variety of topics to help you live your best.

ACCESS ONSITE HEALTH EDUCATION SESSIONS

Participate in a Healthy Activity Event and Earn 6 Points

Healthy Activity Events are abundant in Chicago. You can find a variety of local fun runs, marathons, triathlons, mud races, cycling events and more (partial list here). Whether you are planning to complete your first or your fiftieth event, you can get Chicago Lives Healthy credit. To earn six points toward your ongoing quarterly complete the Healthy Activity Form after you complete your event.

IMPORTANT NOTE: All event participants assume all risks and costs including, but not limited to entrance fees, class fees, disease, physical injury or other injury arising from participation in the event(s) and activities, including but not limited to events and activities such as exercise classes, races, walks, contests and sporting events. Your participation is a release and waiver of liability of your employer and Healthways, and is your agreement that you have read and accepted the terms of this liability release and waiver. Participants waive the right to sue the employer and Healthways for any and all losses and damages that arise from any injury to them or to property.

ACCESS HEALTHY ACTIVITY

Complete a Telephonic Coaching Session, and earn 6 points.

(Eligibility for telephonic coaching is determined by a participant’s biometric screening and Well-Being 5 Assessment results. Not all participants will qualify for telephonic coaching.)

Telephonic health coaching is a voluntary program that provides personal support from a health coach to help individuals with moderate to high health risk factors overcome the challenges that stand in the way of their good health. Based on your responses to the questions you completed on the Well-Being 5 Assessment™ and the values presented from your biometric screening you may be offered an opportunity to participate in the telephonic health coaching program when you complete the Health Advisor Check-In Call.

Make entries into your personal health record and earn 2 points per entry.

You can keep track of your health related activities towards improving your personal health by entering key events into your personal health record on Well-Being Connect. If you get a flu shot, have a dental or vision check-up, get a mammogram or a colonoscopy, you can earn two points by entering the date and type of service. Use your personal health record to keep track of these key events!

ADDITIONAL OPTION AVAILABLE FOR CITY OF CHICAGO PPO PARTICIPANTS ONLY:

Participate in a Care Management Program, and earn 12 points per quarter.

If you are pregnant or have a chronic health condition such as diabetes, cardiac disease, asthma, COPD and participate in one of the care management programs offered such as Taking Control of Your Health or Telligen Maternity or Condition Management Programs, you can earn 12 wellness participation points each quarter while participating in these health management programs. You may also qualify for reduced co-payments for certain drugs to help manage your condition.

If you don't yet have diabetes or heart disease but your biometric screening results indicate that you are at high risk of developing these conditions, Telligen can help you reduce your risks with a one-on-one coaching program. If you participate in the Telligen coaching program you will earn twelve points per quarter.

During your Health Advisor check-in call, the Health Advisor will let you know if you would be a good candidate for these programs.

EARN 12 WELLNESS POINTS EACH CALENDAR QUARTER

All participants in the Chicago Lives Healthy program will be required to earn 12 new points during each quarter starting with the third quarter of 2016 (July through September, 2016) by completing wellness program activities. You can earn all 12 quarterly points at one time if you wish!

Here are the ways that you can earn participation points:

Complete Well-Being Connect online activities and earn 1 point for each activity completed.

Well Being Connect™ (WBC) is a dynamic online portal designed to inspire improved health and well-being. The online portal will help you live more fully by getting to know you better, helping you set goals and connecting you to support to achieve your goals. Well Being Connect™ is a convenient and effective way to earn 12 points at one time if you wish. For example, you can track your exercise or food use, create a personal journal to record your thoughts and actions about your health or read recipes and articles tailored to your personal needs.

ACCESS WELL-BEING CONNECT

Attend an Onsite Health Education Session, and earn 6 points.

One of the best parts of the Chicago Lives Healthy wellness program is access to free, top-quality health education sessions for participants. Whether you’re setting goals to lose weight, managing an ongoing lifestyle issues like quitting smoking or just want to learn more about the Chicago Lives Healthy wellness program, a health education session allows you to focus specifically on your questions, needs and goals with a health coach. These short, informative sessions cover a variety of topics to help you live your best.

ACCESS ONSITE HEALTH EDUCATION SESSIONS

Participate in a Healthy Activity Event and earn 6 Points.

Healthy Activity Events are abundant in Chicago. You can find a variety of local fun runs, marathons, triathlons, mud races, cycling events and more (partial list here). Whether you are planning to complete your first or your fiftieth event, you can get Chicago Lives Healthy credit. To earn six points toward your ongoing quarterly participation, complete the Healthy Activity Form after you complete your event.

IMPORTANT NOTE: All event participants assume all risks and costs including, but not limited to entrance fees, class fees, disease, physical injury or other injury arising from participation in the event(s) and activities, including but not limited to events and activities such as exercise classes, races, walks, contests and sporting events. Your participation is a release and waiver of liability of your employer and Healthways, and is your agreement that you have read and accepted the terms of this liability release and waiver. Participants waive the right to sue the employer and Healthways for any and all losses and damages that arise from any injury to them or to property

ACCESS HEALTHY ACTIVITY

Complete a Telephonic Coaching Session, and earn 6 points.

(Eligibility for telephonic coaching is determined by a participant’s biometric screening and Well-Being 5 Assessment results. Not all participants will qualify for telephonic coaching.)

Telephonic health coaching is a voluntary program that provides personal support from a health coach to help individuals with moderate to high health risk factors overcome the challenges that stand in the way of their good health. Based on your responses to the questions you completed on the Well-Being 5 Assessment™ and the values presented from your biometric screening you may be offered an opportunity to participate in the telephonic health coaching program when you complete the Health Advisor Check-In Call.

Make entries into your personal health record and earn 2 points per entry.

You can keep track of your health related activities towards improving your personal health by entering key events into your personal health record on Well-Being Connect. If you get a flu shot, have a dental or vision check-up, get a mammogram or a colonoscopy, you can earn two points by entering the date and type of service. Use your personal health record to keep track of these key events!

ADDITIONAL OPTION AVAILABLE FOR CITY OF CHICAGO PPO PARTICIPANTS ONLY:

Participate in a Care Management Program, and earn 12 points per quarter.

If you are pregnant or have a chronic health condition such as diabetes, cardiac disease, asthma, COPD and participate in one of the care management programs offered such as Taking Control of Your Health or Telligen Maternity or Condition Management Programs, you can earn 12 wellness participation points each quarter while participating in these health management programs.You may also qualify for reduced co-payments for certain drugs to help manage your condition.

If you don't yet have diabetes or heart disease but your biometric screening results indicate that you are at high risk of developing these conditions, Telligen can help you reduce your risks with a one-on-one coaching program. If you participate in the Telligen coaching program you will earn twelve points per quarter.

During your Health Advisor check-in call, the Health Advisor will let you know if you would be a good candidate for these programs.

Failure to participate in ongoing quarterly participation beginning in April 1, 2015, will result in a $50 per participant per month increase in your monthly employee health care contribution. You will receive several notices that quarterly points have not been achieved before an increase in your monthly employee health care contribution is applied. Individuals who elect not to participate will not be eligible to enroll in the wellness program again until the 2016 benefits open enrollment period in the fall of 2015.
