STEP 1:

SCHEDULE AND ATTEND A BIOMETRIC SCREENING by March 10, 2018

All eligible employees and covered spouses/domestic partners/civil union spouses will be required to complete a biometric screening for the 2018 wellness program year.

What is a Biometric Screening?

A biometric screening includes tests to measure your cholesterol, triglyceride and glucose levels. During your biometric screening appointment, a health care professional will also determine your blood pressure, height, weight and waist measurements.

What options do I have for completing my Biometric Screening? *

You can complete your biometric screening in one of two ways:

A. Schedule an appointment and attend an onsite screening at a community or work location such as City Hall or select Chicago Public Libraries or select Chicago Park District facilities; OR,

B. Schedule and attend an appointment with your doctor.

*Please Note: Walgreens Pharmacies no longer provide Biometric Screening tests. Walgreens Pharmacies are not an option for you to complete your biometric screening for the 2018 wellness program.

A. Onsite Biometric Screening Instructions

Beginning December 1, 2017, you can schedule your free onsite biometric screening at select locations. If you choose to complete an onsite screening during work hours, please consult your supervisor prior to scheduling to ensure proper coverage on the job. 1. You must schedule an onsite screening appointment; walk-ins cannot be screened.

2. Please arrive at least 15 minutes in advance of your scheduled appointment time to complete necessary sign-in activities.

3. The screening will typically take about 15 minutes.

Appointments are available from January 17, 2018 through March 10, 2018. If you attend one of these onsite screenings, your results will be automatically sent to the Chicago Lives Healthy wellness program.

B. Doctor's Office Biometric Screening Instructions

Beginning on December 1, 2017, you can also schedule an appointment with your physician’s office to complete your biometric screening. Follow these steps below:

1. Call your doctor to schedule an appointment.

2. Download the Personalized Physician Form. Bring your Personalized Physician Form with you to your doctor's office.

3. Let your doctor know the Personalized Physician Form must be completed and faxed to 615-349-2344 by March 10, 2018.

Here is some additional Information about Biometric Screenings completed in your doctor's office:

• Healthways will accept biometric screening results if the test results are no more than six months old (no results collected prior to July 1, 2017 will be accepted).

• Biometric screenings done at your doctor’s office are subject to your current medical plan benefits. HMO members must see their Primary Care Physician (PCP) and PPO members must use an in-network provider.

• Your biometric screening results must be submitted on your Personalized Physician Form. If your physician submits biometric results on any other type of form, the results cannot be accepted and your enrollment Step 1 will be considered incomplete.

Personalized Physician Forms are available beginning December 1, 2017. Click the button below and enter you legal first name, legal last name, date of birth, and gender. Printout your Personalized Physician Form and take it with you to your doctor’s office.

What if I don't complete a Biometric Screening?

If you do not complete your biometric screening with results submitted by March 10, 2018, the City of Chicago employee will incur a $50 per non-participant increase in his/her monthly health care contribution.

STEP 2:

COMPLETE A WELL-BEING 5 ASSESSMENT (WB5) by March 10, 2018

All eligible employees and covered spouses/domestic partners/civil union spouses must complete a Well-Being 5 Assessment (WB5). You have from January 4, 2018 through March 10, 2018 to complete your online WB5 for the 2018 plan year.

The Well-Being 5 Assessment (WB5) measures your current well-being and is used to help you and your Health Advisor identify potential areas of focus for your personal 2018 Wellness Program goals.

The WB5 is a simple, confidential questionnaire that looks at your lifestyle habits and current health status to see what may be helping or hurting your overall well-being. You will answer a few focused questions and get instant feedback in your personal Well-Being 5 Snapshot, including suggestions on ways to increase your well-being.

The questions are simple to understand and easy to answer. It will take about 15 minutes to complete the questionnaire. You can complete your online WB5 prior to completion of your biometric screening or before your biometric screening results are processed and recorded on Well-Being Connect.

Important Information:

1. Please complete the online WB5 even if your biometric results are not present on Well Being Connect. Once you begin the WB5, if your biometric screening results are not present, you will be prompted to input your height and weight measurements and then continue to the next page. When your biometric screening results are processed, your WB5 and your Well-Being 5 Snapshot will be updated with your results. Entering your biometric screening results in the WB5 does not satisfy the requirement to obtain a screening.

2. RETURNING USERS: Log on to Well-Being Connect (WBC), using your registered username and password. You will then be prompted to complete the WB5. When you click the Finish button, you will immediately receive your Well-Being 5 Snapshot.

2. NEW USERS: Go to WBC and click the “Sign Up” button. You will be prompted to register and create a username and password. Please document your username and password as you will need this information to access Well-Being Connect in the future. You will then be prompted to complete the WB5. When you click the Finish button, your assessment is complete and your Well-Being 5 Snapshot will be available. Within 24 hours you will receive an email indicating you have access to Well-Being Connect to create your Well-Being Plan to help you set and achieve your health goals. When your actual biometric screening data is loaded, you will be notified via email and you can log in to review your new Well-Being 5 Snapshot.

Do you need help completing your Well-Being 5 Assessment (WB5)?

Health coaches in the Chicago Lives Healthy wellness program are available to help you complete your Well-Being 5 Assessment. Here are two ways to sign up for a Well-Being Help Session:

1. Click here to sign up for a one-on-one WB5 Help Session.

2. Call Healthways at 1-866-556-7671 and sign-up for a WB5 Help Session through a customer service representative.

STEP 3:

TAKE THE CALL FROM A HEALTH ADVISOR by April 30,2018

All eligible employees and covered spouses/domestic partners/civil union spouses must complete a Health Advisor Check-In Call by April 30, 2018. A Healthways Health Advisor will call you when they have the results from your biometric screening and your WB5 on file for 2018. Health Advisor calls can be completed from January 4, 2018 through April 30, 2018.

WHAT IS THE HEALTH ADVISOR CHECK-IN CALL?

The Health Advisor Check-In Call is an outbound call to you from one of Healthways' certified health professionals. During this 15 minute call, the Health Advisor will review your biometric screening and your Well-Being 5 Assessment results (WB5) with you. The Health Advisor will talk with you about your health and well-being and if you wish, develop realistic health or lifestyle improvement goals with you.

Please update your primary phone number in Well-Being Connect so that the Healthways Health Coach can call you to complete the Health Advisor Check-In Call.

IMPORTANT INFORMATION ABOUT YOUR HEALTH ADVISOR CHECK-IN CALL:

1. Individual Health Advisor Check-In calls will be made to each wellness participant.

2. If you have completed your biometric screening and your online Well-Being 5 Assessment and do not receive a call from a Healthways Health Advisor, please call 1-866-556-7671. Please also check to see if the phone number that you have provided is correct.

IMPORTANT INFORMATION:

Healthways' Health Advisors will call you at the phone number you listed in the Well Being Connect portal to complete your Health Check-In Call. Three outbound call attempts will be made to you by Healthways. If Healthways’ Health Advisors have called you three times with no response, they will send you a letter indicating that you MUST contact a Health Advisor to complete your Health Advisor Check-In Call. Call Healthways at 1-866-556-7671 to complete your Health Advisor Check-In Call before April 30, 2018.
